FALL REGISTRATION INFORMATION Please read through all information before submitting your online registration form! *Please submit one form per child . * First Month's and June's Tuition payment is due at the time of registration. *There is a $20 registration fee for each student. *You will be billed at the address submitted on the registration form. You may bring this payment to the studio or mail it to: Create Dance Center, Ltd. 143 Merrick Road Amityville, NY 11701 *Please note: We currently accept Cash or Check. Checks should be made payable to: Create Dance Center,Ltd. *Information regarding tuition can be obtained by submitting the form on the questions page. Please make sure to ask for the tuition chart and a copy will be emailed to you upon request.
